AWANA Application

To download the online Application Form right click on the link then click on Save target as. This will let you save the application file to your computer. Next open the file fill in the contact information and answer the questions. Save the file, then attach the file to a email.

Instructions:
Complete a registration form for each child and a fee form for each family, name each file with a unique name and e-mail them all back to Kathy at sacnurse@surewest.net

After you have completed the online application and fees form you may send in your payment securely online using PayPal.
